Acceptable Forms of Identification:
- A Driver's License issued by the Texas DMV
- A United States Passport
- Photo Identification Card issued by another state, which includes a physical description of the person, signature of the person, and an identifying number.
- Driver's License issued by another state or by a Canadian or Mexican public agency authorized to issue DL.
- A Passport issued by a foreign governments, provided that it has been stamped by the U.S. Immigration or Naturalization Service or the U.S. Citizenship and Immigration Services.
- Military Identification Card
ALL ID's provided MUST be current or have been issued within the last five years.
If no such ID is available and cannot be reasonably obtained, the signer (you) may have TWO CREDIBLE WITNESSES present who know you and can attest to who you are. Thus they provide the above acceptable forms of ID and sign the notary's journal along with you "the signer."

I am unable to provide these services if:
If I am called to a location and the signer is unable to sign, the document is not complete, or other questionable circumstances prevail, you will still be liable for the travel/time fee as agreed when booking the appointment.
- The person requiring notarization does not have proper and valid ID and cannot provide witnesses with proper and valid ID to confirm the signer’s identity.
- The person requiring notarization is not present to sign.
- The person is not mentally competent to sign.
